Online Application Instructions

In order to access and submit an Online Application you need to first complete an Information Request Form. 点击这里 to access the Information Request Form.
If you have already completed the Information Request Form you DO NOT need to complete the form again. Please proceed to STEP 2 as you are already in our system.
步骤2
After submitting the Information Request Form you will receive an email with instructions to complete the setup of your login account. Please allow up to two business days to receive this information.
步骤3
Once your username and password have been configured, click here to log into the site and start your application.
Click on your daughter’s name on the top left corner and view the “Checklist” under “澳门永利APP官网 Progress”.
Click on the “Fill Out” button to start the online application.
To check whether 完美的心 has received your daughter’s application paperwork, you will need to log in, click on your daughter’s name on the top left corner, and view the “Checklist” under “澳门永利APP官网 Progress”.
